  • Government mail from the federal, state, county, or city; or
  • The local sheriff’s department filled out the FDLE registration form.

Customers who do not have any of the above documents in their name can provide two proofs of home address from a parent, stepparent, legal guardian, or other person with whom they live, as well as a Certification of Address form.

Customers are not needed to produce these documents for successive renewals under the REAL ID Act unless their information changes (e.g., name, address). If the customer is applying for a Commercial Learner’s Permit (CLP) or a Commercial Driver’s License (CDL), they must provide proof of residency documentation as indicated below.

Applicants for the CLP or CDL must show two evidence of Florida residency in their name. Applicants can submit either two List A proofs or one List A proof and one List B proof.

1. A current lease or rental agreement for the residence with a period of at least 6 months.

2. A copy of your current voter registration card in Florida

3. Registration or title of a vehicle or vessel in Florida

4. A copy of your most recent homeowner’s insurance policy or a bill

Applicants for a CLP or CDL who are unable to provide a proof of home address from list A must submit a Declaration of Domicile submitted with the Clerk of the Circuit Court in accordance with Florida Statutes section 222.17 AND one proof of residential address document from list B.

A Certification of Address (HSMV-71120) cannot be used for licensure by CLP or CDL applicants.

Customers who have legally changed their name due to marriage, divorce, or a court order must present the original or a certified copy of their marriage certificate, divorce decree, or court order. No photocopies will be accepted. The state of Florida requires marriage certificates to be recognized. Marriage certificates issued in Florida or another state in conformity with those states’ laws are generally accepted by the state of Florida. Marriage certificates issued by churches are not acknowledged.

Marriage certificates and divorce decrees can be obtained from the clerk of court in the county where they were first filed.

We will not be able to approve your application for a Freedom Pass if you do not submit verification of your name, age, or address.

A recent passport-sized color photograph (within the last 12 months) is required for all Freedom Pass applications.

Your photo should be:

  • 45mm x 35mm in color and size
  • A current genuine likeness of yourself, sans a cap, facing forward.

We will not be able to accept your Freedom Pass application if your photograph does not fulfill the aforementioned conditions.
